Health & Sanitation Practices
Illness/COVID-19 Practices:

We are taking extra precautions with the intake of each client, health history review, sanitation, and disinfecting practices. Your Provider at Smoothe & Co. will be wearing face masks and require clients to wear face masks upon entering the building.
We only allow the client that will be serviced in the treatment (no children allowed).
At Smoothe, the beds, surfaces, doorknobs, light switches, and payment systems will be thoroughly wiped down after every client. In addition, the floors, cabinets, and all other areas will be thoroughly cleaned after every shift.
Consent for Treatment
Please read and verbally agree to the following certification statement before the start of your service:
I, ______________, affirm that I, as well as ALL household members, have not been diagnosed with ANY illness/COVID-19 in the last five days.
I have not had any of the following symptoms in the last five days:
- Fever
- Cough
- Shortness of breath
- Sore throat
- Body Aches
- Extreme exhaustion
- Diarrhea and vomiting
- Loss of sense of smell or taste
I have not been around anyone exhibiting any of these symptoms within the last five days.
I understand that this Business (Smoothe & Co.) and my Esthetician(s) cannot be held liable for any virus exposure or other contagion. Furthermore, I agree not to have the Business or its employees accountable if I contract COVID-19 or any other contagion. I understand there is no way to safely social distance because aesthetics involves touch and close physical proximity. I have decided to come here of my own free will, and I am aware of the risks involved in receiving treatment at this time.
Smoothe & Co. and all employees agree to abide by and adhere to the same standards. They also affirm that they have improved and expanded their sanitation practices and protocols. They adhere to State-mandated guidelines and have always practiced standard and universal precautions.
